Standard Operating Procedure: Institutional Facilities Health & Safety Plan (IFSP)
1.0 Purpose & Institutional Scope
This plan establishes health, safety, and physical security standards for HWB Cleaning Services LLC personnel operating inside occupied public authority facilities, transportation centers, higher education campuses, and municipal complexes. It addresses unique environmental risks including pedestrian traffic, student interactions, biological fluid hazards, secure facility controls, and chemical management in multi-facility campuses (such as the North Texas Tollway Authority Ancillary Facilities and Collin College).
2.0 Scope
This procedure applies to all custodial technicians, day porters, floor care specialists, and operations managers assigned to recurring commercial, municipal, and institutional contracts.
3.0 Bloodborne Pathogens & Biohazard Exposure Control Plan (OSHA 29 CFR 1910.1030)
Restroom sanitation, trash removal, and common area cleaning present potential risks of contact with blood, saliva, or other potentially infectious materials (OPIM). HWB enforces universal precautions:
- Universal Precautions: All human blood and bodily fluids are treated as if known to be infectious for HIV, Hepatitis B (HBV), and other bloodborne pathogens.
- Mandatory Personal Protective Equipment (PPE): Technicians must wear disposable nitrile gloves when servicing restrooms, pulling trash, or wiping high-touch fixtures. Heavy-duty puncture-resistant utility gloves must be worn when cleaning toilets and urinals.
- Biohazard Spill Response: Every facility closet must be equipped with an OSHA-compliant Body Fluid Clean-up Spill Kit (containing disinfectant absorbent powder, disposable scoop, disinfectant towelettes, biohazard bag, and splash goggles). Liquid spills must be absorbed, treated with EPA-registered hospital disinfectant for full dwell time, and placed in a sealed red biohazard disposal bag.
- Sharps & Needles Awareness: Never push trash down into a receptacle with hands or feet. If a discarded syringe or needle is discovered, technicians must not touch it directly. Use a mechanical gripper or dustpan and broom to transfer the needle into an approved puncture-proof sharps container.
- Hepatitis B Vaccination: HWB offers the complete Hepatitis B vaccination series free of charge to all employees who have potential occupational exposure to bloodborne pathogens. Technicians who decline must execute a signed OSHA Declination Form.
4.0 Public Pedestrian Safety & Slip, Trip, and Fall Prevention
Wet floor maintenance in occupied buildings represents the primary source of liability and injury in institutional environments:
- Caution Wet Floor Signage: High-visibility bilingual (English/Spanish) "Caution Wet Floor" cones must be positioned at all entry points to a wet area before mopping or scrubbing begins. Signs must remain in place until the floor surface is 100% dry.
- Half-Hallway Mopping Procedure: In corridors, lobbies, and academic wings, technicians must mop only one half of the corridor width at a time, leaving the other half dry, clear, and unobstructed for public and emergency egress.
- Electrical Cord Management: Cords for backpack vacuums, burnishers, and carpet extractors must be kept behind the operator, never stretched across open doorways or stairs. Heavy-duty yellow rubber cable ramps must be placed over cords running across active walkways.
- Outdoor Entrance Walk-Off Mats: During rainy or icy weather, entrance mats must be checked hourly and water build-up extracted immediately to prevent slick transitions onto terrazzo or polished concrete.
5.0 Chemical Dilution Systems & Janitorial Closet Management (OSHA 29 CFR 1910.1200)
- Closed-Loop Dilution Stations: Cleaning chemicals (neutral floor cleaners, peroxide general cleaners, multi-surface disinfectants) must be mixed using wall-mounted closed-loop dispensers that automatically meter water and chemical ratios, eliminating manual pouring hazards.
- Secondary Container Labeling: Every spray bottle or bucket must have a factory-printed GHS secondary label displaying the chemical name, manufacturer, and hazard warnings. Writing on bottles with markers is strictly forbidden.
- Janitorial Closets & Slop Sinks: Closets must be kept locked, clean, and organized. Slop sinks must be drained and wiped down after every shift. Chemicals must be stored off the floor on wire racks with incompatible chemicals physically separated.
- Eye Wash Stations: Chemical storage areas must have clear, unobstructed access to an emergency eye wash station capable of providing a 15-minute continuous flush. Stations must be visually checked weekly.
6.0 Commercial Floor Care Machinery Protocols
- Auto-Scrubbers (Walk-Behind & Ride-On): Operators must travel at walking speed, look in the direction of motion, and slow down around blind corners. Headlights and flashing amber beacon lights must remain turned on during operation in active facilities.
- Battery Charging Safety: Scrubbers equipped with wet-cell lead-acid batteries must be charged only in well-ventilated areas away from sparks or open flames to prevent hydrogen gas accumulation. Eye protection and rubber gloves must be worn when checking electrolyte levels.
- High-Speed Burnishers: Technicians must inspect the pad centering lock and power cord before each use. Never leave a plugged-in machine unattended.
7.0 Facility Security, Access Control & Institutional Governance
Public authority contracts (such as NTTA toll plazas and administrative centers) and educational campuses require strict adherence to physical security protocols:

8.0 Custodial Ergonomics & Zero-Injury Mechanics
- Backpack Vacuum Ergonomics: Technicians must adjust the waist belt first (so that 80% of the weight rests on the hips, not the shoulders) before tightening shoulder straps. Maintain an upright posture and use arm sweeping motions rather than bending at the waist.
- Mop Bucket Mechanics: Never lift full 5-gallon mop buckets. Use the bucket's pour-spout and wheels. Empty half-full buckets more frequently to reduce lift weights below 25 pounds.
- Trash Barrel Tipping: When emptying heavy 44-gallon Brute containers into exterior dumpsters, utilize 2-person team lifts or hydraulic trash lifts. Never twist the spine while lifting.
9.0 Emergency Action Plans in Occupied Facilities
- Active Threat (Run / Hide / Fight): Know two exit paths from every work zone. If an active threat occurs, alert others, evacuate immediately if safe, or shelter in locked utility closets, silencing radios and phones.
- Fire Alarms: Evacuate immediately upon alarm sounding. Direct any nearby occupants toward lighted exit signs. Report immediately to the campus or agency muster point.
- Severe Weather & Tornado: Guide crews to interior restrooms or designated emergency storm shelters located on the ground floor away from exterior glass.
- Agency Dispatch Coordination: In an emergency, immediately notify local 911, then contact the NTTA Operations Center or Campus Police dispatch desk using official contact numbers.
10.0 Verification (Zero-Defect Audit Check)
- Confirm that all secondary spray bottles have intact GHS bilingual labels.
- Confirm that all janitorial closets remain locked when unattended.
- Confirm that Body Fluid Clean-up Spill Kits are fully stocked in every primary storage closet.
- Verify that all active team members have signed Hepatitis B vaccination acceptance or declination forms.
- Ensure that all assigned technicians display verified photo ID badges at all times.
