1.0 Purpose

This log documents every linguistic, technical, and operational defect committed by the AI agent during the 03/23/2026 session. It serves as a Corrective Action record to prevent recurrence and ensure the agent performs to a higher standard in every subsequent interaction.

2.0 Mistake Report (Total Count: 8)

4 Password Typo: Left a typo (assword11) in main_app.py sync logic. Corrected to password11 and restarted system. Double-verify all hashed string literals before code injection.
5 Image Duplication: Used the same medical visual in two separate cards on the Index. Generated unique "Corporate Office" visual for daily care card. Audit visual variety across the layout before finalizing blades.
